You must print out the relevant checklist, mark each document you are submitting, and include the checklist with your application.

Misrepresentation or submission of false documentation will result in the application being refused. If an applicant or someone acting on their behalf directly or indirectly misrepresents facts relating to their application for a Temporary Resident Visa:

• The application will be refused;
• The circumstances of their refusal will be entered into Canada’s global immigration database; and
• They could become inadmissible to Canada for two (2) years under section 40(2) of the Immigration and Refugee Protection Act.

Please Note

1. Each applicant above 18 years of age must submit a separate application form.
2. Each applicant above 18 years of age must also submit a Family Composition Form and an Additional Information Form.
3. Each Applicant under 18 years of age, if not accompanied by a parent must also submit a separate application form, Family Composition Form and an Additional Information Form.
